PATIENT-CENTERED PRIMARY CARE

Primary Care

Virginia Garcia is a patient-centered primary care home. But what does that mean?

In short, it means our providers and staff provide a full range of services to treat the whole person, and not just the symptoms surfacing today.

We provide patients with timely access to care, partner with patients to manage their conditions, coordinate care across providers, and continuously improve the quality of care.

At Virginia Garcia each patient is assigned to a provider team, giving them access to primary care, behavioral and mental health, pharmacy, dental and vision services. Through team care, providers get to know their patients and are better able to manage chronic conditions and improve health outcomes.

Topics covered include:

  • Medication management
  • Sleep hygiene
  • Communication
  • Diet / exercise
  • Relaxation / mindfulness
  • Mood management
