We have listed the vaccine by clinic location. Select the clinic that is closest, or most convenient.  You do not have to be a patient of the clinic where you receive the vaccine.

Appointments are required at most locations, although some clinics do offer walk-in opportunities. To see those times and locations, please visit our Vaccine Event page.

Virginia Garcia will receive the recently approved COVID-19 Vaccine for children 6 months to 4 years old by June 30. We will provide a scheduling options specifically for this age group. 

Please do not schedule a vaccination appointment for a child under 5 if there is not an option for under 5.

You can schedule your vaccination appointment at any of the following clinics. You do not need to be a patient, have insurance or show identification. The COVID-19 vaccine is free, regardless of insurance.

Pfizer vaccine

5 years and older. Select the dose and then location from the options below.

If you are bringing a minor under the age of 12 to be vaccinated and you are not the legal guardian or parent, you will need a signed copy of the form below. Virginia Garcia cannot vaccinate a minor who is not accompanied by a legal guardian or parent without this form.

Parent Consent for minor vaccine

If you are a child ages 12-14 years and cannot bring a legal guardian or parent with you to receive the vaccine, you will need one of the below forms signed and bring it with you to the event.

First Dose
6 months and older

Second Dose
5 years and older. Second doses must be given at least 21 days after first dose.

Third Dose
For those with weakened immune systems - 12 and older - to be given no sooner than 28 days after the second dose is received

1st and 2nd Booster Dose
1st Booster Dose: 12 years and older. 1st Booster dose may be given no less than 5 months after the second or third dose.
2nd Booster Dose: Available to those 50 years and older or those 12 years and older that are immunocompromised. 2nd Booster dose may be given no less than 4 months after the first booster dose.

MODERNA vaccine

12 years and older. Select the dose and then location from the options below.

If you are bringing a minor under the age of 12 to be vaccinated and you are not the legal guardian or parent, you will need a signed copy of the form below. Virginia Garcia cannot vaccinate a minor who is not accompanied by a legal guardian or parent without this form.

Please note, we are currently not offering the vaccine to children under the age of 5. If you would like to have your child 6 months - 4 years of age vaccinated, please select from the options available under Pfizer.
