This means integrated care where you can go to one place for all your health care needs and see providers that know who you are and are invested in helping you be healthy.
A primary care home partners with patients to provide:
- Timely and enhanced access to care
- Management of existing health conditions
- Coordinated care across providers
- Engagement in continuous quality improvement
At Virginia Garcia, each patient is assigned to a provider team that gives them access to primary care, behavioral and mental health, pharmacy, dental and vision services. Through team care, providers get to know their patients and are better able to manage chronic conditions and improve health outcomes.
Virginia Garcia has achieved a Tier 4 rating as a Patient Centered Primary Care Home and recognition as “a statewide model for providing authentic team-based care” by the Oregon Health Authority.
