I am pregnant and want to enroll at the Reproductive Health Clinic.
If you are pregnant, you do NOT need to sign up as a patient with the health center first. You can call the clinic directly at 503- 597-4513 and they will help you schedule an appointment and get signed up. Virginia Garcia will also assist you in OHP enrollment as well as selecting a primary care doctor.
I am not pregnant, but would like my care to be at the Reproductive Health Clinic.
If you are already a patient of Virginia Garcia, you may have your annual women’s checkup at the Reproductive Health Clinic. Simply ask our scheduling assistant to make your next appointment at the clinic.
Do I have to be a patient at Virginia Garcia to use the Reproductive Health Clinic?
If you have not signed up for the Oregon Health Plan (OHP), member services will help you do this as well. You do not have to wait to qualify for OHP to sign up at Virginia Garcia.
Once you have signed up with Virginia Garcia, you can schedule your annual exam at the Reproductive Health Clinic.
How do I sign up as a patient of Virginia Garcia?
To enroll as a patient of Virginia Garcia or learn more about our Reproductive Health Clinic services, call 503-597-4513.
What do I need to do to enroll or renew in OHP?
In order to enroll or re-enroll in OHP, Virginia Garcia will need the following information:
- Renewal packets received, including the attached letter.
- Identification Cards (ID).
- Social security cards for everyone.
- Legal Permanent Resident cards or Employment authorization card if applicable.
- Income verification for everyone in the household for previous and current month.
- Most recent W-2.
- Copy of your most recent taxes.
- If you have other tax dependents besides children, you will need to know their full name and DOB.
- Insurance cards, OHP/insurance through employers (if applicable or renewing).
- Address and phone number of employers.
