At Virginia Garcia, we put you at the center of everything we do. That’s why we are proud to be recognized as a Patient Centered Primary Care Home (PCPCH). This means we focus on building strong relationships with you, your provider, and your care team to help you stay healthy.
Your Care Team
When you become a patient at Virginia Garcia, we assign you a primary care provider (PCP). Besides your PCP, the team also includes nurses, medical assistants, and others who work together to support you. The team members include doctors, nurses, medical assistants, and others who collaborate to support your health.
What Your Care Team Does for You
Your care team is here to help you with all parts of your care, not just when you are sick. Two key people on your team are the Team Assistant and the Panel Care Coordinator. They work closely with your provider to make sure nothing gets missed.
Team Assistants Can:
Help schedule and coordinate visits to specialists
Make sure outside medical records are sent to and from Virginia Garcia
Answer questions about tests like X-rays, MRIs, or CT scans
Panel Care Coordinators can:
Respond to MyChart messages and route non-urgent questions to the right team member
Assist with forms, paperwork, and equipment requests
Answer questions about prescriptions
