When you visit Virginia Garcia, you’ll see the same team of providers each time. They’ll get to know you and your health needs.
We want you to feel safe and welcome. Every new patient receives a full check-up. We’ll look at the things that may affect your health and help you take control of your wellness.

What to Expect at Your First Visit

Please arrive 30 minutes before your appointment and bring the items listed below. This gives us time to:

  • Check in
  • Complete any paperwork
  • Verify insurance (if you have it)
  • Learn if you qualify for a discount
  • Get an interpreter (if needed)

Your Care Team will get to know you and work with you to support your health and wellness. We provide services in more than 62 languages.

Prepare for Your Appointment

What to Bring

Photo ID – A driver’s license, work or school ID, or any card with your picture.

Health Insurance Card – If you have health insurance, bring your card.

All Medications – Bring the bottles or a list of everything you take, including vitamins and supplements.

Vaccine Record – For you or your child, if available.

Medical History – Any past or current health issues, surgeries, family health conditions, or immunizations.

Proof of Income – Such as a pay stub or a letter from your employer. This helps us see if you qualify for a discount. If you don’t have income, let us know during your visit.

$20 Payment – If you are paying for your visit yourself.

If You Are Changing Clinics

If you are coming to Virginia Garcia from another doctor or clinic:

  • Bring a copy of your past medical records if you have them
  • Ask for a Release of Information form so we can request your records if needed:

Already a Patient?

If you are already a Virginia Garcia patient, please bring the following to every visit:

Health Insurance Card
If you have health insurance, bring your current insurance card.

Photo ID
A driver’s license, work or school ID, or any card with your picture.

Medication List
Bring any new medications you’ve started since your last visit.

Tip: Bring the medication bottles if you can, this helps us confirm the name and dosage.
